RESUMO

Gardner syndrome (GS) is a rare autosomal dominant disorder that can present with craniomaxillofacial abnormalities. The identification of osteomas or craniomaxillofacial abnormalities can therefore serve as a marker of this condition, facilitating early referral and diagnosis. A 17-year-old female with GS was referred for the management of severe limited mouth opening, causing a major problem for routine endoscopy to monitor the gastrointestinal alterations of GS. Clinical and radiological evaluations showed multiple osteomas in the mandibular angle, condylar and coronoid regions bilaterally and maximum mouth opening of 8 mm. The patient underwent surgery for osteoma removal and bilateral customized alloplastic total temporomandibular joint replacement (TMJ-TJR). At the 2-year follow-up, the patient showed improvements in quality of life, with a maximum mouth opening of 34 mm, allowing routine upper endoscopy to be performed. This is the first report of GS, a rare and challenging craniomaxillofacial abnormality, treated with TMJ-TJR. A comprehensive overview of the patient's clinical presentation, diagnostic assessment, treatment planning, and outcomes is provided.

Introducción: La teoría de la mente (TM) está involucrada en la cognición social, ya que evalúa nuestra capacidad para atribuir estados mentales a los demás con el fin de predecir y explicar el comportamiento. En la bibliografía, se ha observado que los niños con trastorno por déficit de atención/hiperactividad (TDAH) muestran algunas alteraciones en la TM en comparación con los niños sin problemas de neurodesarrollo. Nuestro objetivo en este estudio fue comparar la TM en dos grupos: niños en edad escolar con desarrollo normal y niños en edad escolar con TDAH. Sujetos y métodos: Se reclutó a 35 niños con edades comprendidas entre los 6 y los 12 años: 17 con TDAH y 18 sin problemas de neurodesarrollo. La TM se evaluó utilizando un método de evaluación validado para la población portuguesa: Tortuga en la Isla-Batería de Evaluación de Funciones Ejecutivas en Niños. Resultados: Obtuvimos dos grupos comparables en cuanto a datos sociodemográficos. No hubo diferencias significativas entre los dos grupos en cuanto a la TM. Conclusiones: La evaluación de la TM en niños portugueses no reveló alteraciones significativas en esta habilidad cognitiva en niños con TDAH.(AU)

Septic arthritis of the temporomandibular joint (SATMJ) is an uncommon bacterial or fungal infection of the joint space. A 68-year-old man with underlying diabetes mellitus and a history of liver transplant, who was on immunosuppressants, presented to the oral and maxillofacial surgery department of the authors´ institution in Portugal. His main symptoms were arthralgia in the right temporomandibular joint, malocclusion, pre-auricular swelling and erythema. After clinical, laboratory, and imaging evaluations, diagnoses of chronic suppurative otitis media and SATMJ were made. The patient was managed with arthroscopy of the right temporomandibular joint, which allowed joint fluid collection for microbiological examination, lavage, and biopsy. The biopsy sample was positive for Pseudomonas aeruginosa. After surgery, targeted intravenous antibiotic treatment (amikacin) was given for 30 days. No recurrence of any complaints was reported after 12 months of follow-up.

INTRODUCTION: In this prospective ex vivo study, we propose a new technique for the intraoperative examination of retroareolar tissue and describe both surgical excision and pathological methods. We performed a nipple-sparing mastectomy simulation in patients selected to total mastectomy, in order to evaluate the accuracy of these new technique. MATERIALS AND METHODS: A total of 158 total mastectomy specimens from patients affected by ductal carcinoma in situ (n = 15) or invasive ductal carcinoma (stages I, II, or IIIA) (n = 143) were examined. To obtain the entire sample area, the terminal retroareolar milk duct bunch was isolated. Fragments approximately 1.5 cm in length were excised and sectioned in parallel at the base of the nipple using a cold bistoury. Three transverse histological sections (4 µm each) at 200 µm intervals that included the entire isolated fragments were subjected to frozen section examination. The sections were stained with hematoxylin-eosin and were evaluated. The remainder of each fragment was embedded in paraffin and 4 µm sections were subsequently stained with hematoxylin-eosin and examined. RESULTS: There were two false-negative (1.3%) and five false-positive (3.1%) findings among the frozen and paraffin sections analyzed. A statistical analysis of the frozen section examinations showed a sensitivity of 92.0%, a specificity of 96.2%, a positive predictive value of 82.1%, a negative predictive value of 98.4%, and an accuracy of 95.4%. CONCLUSION: The frozen section examination technique described here detected nipple involvement in breast cancer with greater accuracy than the frozen section usually performed by most surgeons.

RESUMO Psidium cattleyanum Sabine, conhecida como “araçá”, é espécie nativa do Bioma Cerrado brasileiro comumente utilizado, segundo levantamento etnobotânico, como planta medicinal para tratar várias doenças tais como: patologias hepáticas, gástricas, lesões teciduais incluindo processos dolorosos. O objetivo deste trabalho foi realizar a triagem fitoquímica com propósito exploratório, investigar a atividade analgésica e antimicrobiana do extrato hidroalcoólico da casca do caule de Psidium cattleyanum Sabine (ECPCS) para apoiar o uso dessa espécie como planta medicinal. Para isso, foram obtidos extratos e frações com solventes orgânicos de polaridade crescente (hexano, diclorometano, acetato de etila e isobutanol) avaliando-se o perfil fitoquímico para determinar as principais classes de metabólitos secundários presentes na espécie. Investigou-se a atividade analgésica pelo teste de contorções abdominais em camundongos induzidas pelo ácido acético (0,6%). A Concentração Inibitória Mínina (CIM) e Concentração Bactericida Mínima (CBM) foram avaliadas através da técnica de microdiluição em caldo contra micro-organismos da microbiota oral. A triagem fitoquímica identificou a presença de taninos, saponinas, flavonoides e terpenos e/ou esteroides. O ECPCS exibiu atividade analgésica periférica nas doses de 200 e 400 mg/kg. O EAC (extrato acetato de casca) o EDC (extrato diclorometânico de casca) desempenharam melhor ação inibitória sobre o crescimento bacteriano de Estafilococos oralis com CIM 100 e 150 respectivamente. O ECPCS desempenhou ação inibitória sobre o crescimento bacteriano. Os resultados dos estudos experimentais comprovaram a presença de compostos secundários tais como, taninos e flavonoides, o que, provavelmente, pode ser associado à atividade analgésica e ao efeito inibitório sobre os micro-organismos testados com o ECPCS, o que justifica o uso medicinal planta.


ABSTRACT The Psidium cattleyanum Sabine, known as “araçá”, is a native species from the Brazilian Cerrado biome, commonly used, according to ethnobotanical surveis, as a medicinal plant to treat several sicknesses such as liver and , gastric diseases and tissue lesions with painful treatments. The aim of this study was to perform an exploratory screening, investigating the analgesic and antimicrobial activity of the hydroalcoholic extract of the Psidium cattleyanum Sabine (BEPCS) stem bark, in order to support the use of this species as a medicinal plant. For that, the BEPCS and its parts were obtained from the extraction with organic solvents of increasing polarity (hexane, dichloromethane, ethyl acetate and isobutanol), evaluating its phytochemical profile in order to determine the main types of secondary metabolites present in the species. The analgesic activity, through the twisting test in mice, was investigated and induced by acetic acid (0.6%). The Minimum Inhibitory Concentration (MIC) and Minimum Bactericidal Concentration (MBC) were evaluated using the microdilution technique in liquid against microorganisms of the oral microbiota. The phytochemical screening identified the presence of tannins, saponins, flavonoids and terpenes and/or steroids. The BEPCS exhibited peripheral analgesic activity at the doses of 200 and 400 mg / kg. The AEB (acetate extract bark) and the DEB (dicloromethane extract bark) had better inhibitory effect on bacterial growth of Staphylococcus oraliswith MIC 100 and 150 respectively. The BEPCS demonstrated an inhibitory effect on bacterial growth. The results of experimental studies have indicated the presence of secondary compounds as tannins and flavonoids, which probably can be associated with the analgesic activity and inhibitory effect on the microorganisms tested with BEPCS, fact that justifies the its application.

Avaliou-se a eficiência do cateterismo posterior do plexo braquial em cães para promover bloqueio motor e sensitivo, por meio de três protocolos anestésicos. Foram utilizados nove cães, machos e fêmeas, sem distinção de raça e idade, com peso variando de 6 a 15kg, distribuídos em três grupos de três animais por grupo. Após a confirmação do correto posicionamento do cateter pela via posterior do plexo braquial por meio do exame radiográfico, foram aplicadas as medicações de acordo com os grupos. No grupo 1, a solução anestésica de bupivacaína 0,5% sem vasoconstrictor, na dose de 2mg.kg-1, foi usada isoladamente. No grupo 2, a solução anestésica de bupivacaína foi associada ao butorfanol na dose de 0,25mg.kg-1 . No grupo 3, o fentanil, na dose de 0,005mg.kg¹, foi associado à solução anestésica de bupivacaína. Não houve diferença estatística significante entre os grupos. Observou-se que a duração dos bloqueios motor e sensitivo foi clinicamente maior no grupo 2. O cateterismo posterior do plexo braquial permite a aplicação de fármacos mais próximos do plexo nervoso, promovendo analgesia complementar nos membros anteriores.


We evaluated the efficiency of the posterior brachial plexus catheterization in dogs to promote motor and sensory block using three anesthetic protocols. We used nine dogs, males and females, without distinction of race and age, weighing 6 to 15kg which were divided into three groups of three animals per group. After confirming the correct positioning of the catheter through the posterior brachial plexus through radiographic examination, the medications were administered according to the groups. In group 1 the anesthetic bupivacaine 0.5% without a vasoconstrictor dose of 2mg.kg-1 was used alone. In group 2 the anesthetic bupivacaine was associated with butorphanol at a dose of 0.25mg.kg-1. In group 3, the fentanyl dose was associated with 0.005mg.kg-1 anesthetic bupivacaine. There was no statistically significant difference between groups. It was observed that the duration of sensory and motor block was clinically higher in group 2. The posterior brachial plexus catheterization allows the application of drugs closer to the nerve plexus promoting additional analgesia in the forelimbs.

Vaccinia virus strains from the family Poxviridae have been frequently isolated in Brazil and associated with outbreaks of exanthematic disease affecting cows and humans. An ELISA IgG was applied to evaluate the seroprevalence of orthopoxviruses in a community located in a rural settlement in the Amazon region, where no orthopoxvirus outbreaks have yet been reported. An overall seroprevalence of 27.89% was found, and it was 23.38% in the non-vaccinated population (smallpox vaccination). These results strongly suggest that orthopoxviruses circulate in this population, and it is the first finding of seropositivity for orthopoxviruses in a population without any previously reported outbreaks.

BACKGROUND: A group of patients with type 2 diabetes mellitus (T2DM) and body mass index (BMI) 20-34 kg/m(2) were submitted to laparoscopic interposition of a segment of ileum into the proximal jejunum or into the proximal duodenum associated to a sleeve gastrectomy. The objective of this study is to evaluate the hormonal changes in the pre- and postoperative period. MATERIALS AND METHODS: Hormonal evaluation was done in 58 patients operated between April 2005 and July 2006. Mean age was 51.4 years (40-66 years). Mean BMI was 28.2 (20-34.8) kg/m(2). All patients had had the diagnosis of T2DM for at least 3 years. Mean duration of T2DM was 9.6 years (3-22 years). Two techniques were performed, consisting of different combinations of ileal interposition (II) associated to a sleeve gastrectomy (SG). The following hormones were assayed in the pre- and postoperative period (mean 16 months) at the baseline and following specific food stimulation (30, 60, 120 min): glucogen-like protein 1 (GLP-1), glucose-dependent insulinotropic peptide (GIP), insulin, glucagon, C-peptide, amylin, cholecystokinin (CCK), pancreatic polypeptide (PPP), somatostatin, peptide YY (PYY), ghrelin, adiponectin, resistin, leptin, and interleukin-6 (IL-6). RESULTS: Thirty patients had II associated to sleeve gastrectomy (II-SG) and 28 had II with diverted sleeve gastrectomy (II-DSG). GLP1 exhibited an important rise following the two operations, especially after II-DSG (p < 0.001). GIP also exhibited an important rise, with both II-SG and II-DSG being equally effective (p < 0.001). Insulin and amylin showed a significant rise at 30 min. Glucagon decreased slightly. CCK measurements were very low after II-DSG. PPP was also slightly altered by the II-DSG. PYY showed an important increase with both operations (p < 0.001). Ghrelin showed a significant decrease following the two operations (p < 0.001). Somatostatin and IL-6 were not affected (p = 0.632). Both leptin and resistin blood levels decreased. Adiponectin showed a slight increase. Mean postoperative follow-up was 19.2 months. Both II-SG and II-DSG were effective in achieving adequate glycemic control (91.2%). CONCLUSIONS: There was a significant hormonal change following laparoscopic ileal interposition. These alterations may explain the promising good results associated to these operations for the treatment of T2DM in the nonmorbidly obese population.

BACKGROUND: The objective of this study is to evaluate the clinical results of the laparoscopic interposition of a segment of ileum into the proximal duodenum associated to a sleeve gastrectomy (II-DSG) in order to treat patients with type 2 diabetes mellitus (T2DM) and body mass index (BMI) 21-29 kg/m2. PATIENTS AND METHODS: The laparoscopic procedure was performed in 69 patients, 22 female and 47 male. Mean age was 51 years (range 41-63 years). Mean BMI was 25.7 (21.8-29.2) kg/m2. All patients had the diagnosis of T2DM for at least 3 years and evidence of stable treatment with oral hypoglycemic agents and or insulin for at least 12 months. Insulin therapy was used by 44% of the patients. Mean duration of T2DM was 11 years (range 3-18 years). Dyslipidemia was diagnosed in 72.5% and hypertension in 66.7%. Nephropathy was characterized in 29% of the patients, retinopathy in 26.1%, and neuropathy in 24.6%. RESULTS: Overall, 95.7% of the patients achieved adequate glycemic control (Hb(A1c) < 7%) without antidiabetic medication. Hb(A1c) below 6% was achieved by 65.2%. Mean postoperative follow-up was 21.7 months (range 7-42 months). Mean postoperative BMI was 21.8 kg/m2. There was no conversion to open surgery. Median hospital stay was 3.4 days (range 2-58 days). Major postoperative complications were diagnosed in 7.3%. There was no mortality. Fasting glycemia decreased from a mean of 218 to 102 mg/dl, postprandial glycemia from 305 to 141 mg/dl, and homeostasis model assessment of insulin resistance (Homa-IR) from 5.2 to 0.77. All associated comorbidities and complications related to T2DM had significant improvement or control. Arterial hypertension was controlled in 91.3%. Macroalbuminuria was no longer observed. Microalbuminuria resolved in 87.5% of patients. Hypercholesterolemia was normalized in 95% and hypertriglyceridemia in 92% of patients. CONCLUSIONS: Laparoscopic II-DSG was an effective operation in controlling T2DM in a nonobese (BM < 30 kg/m2) population. Associated diseases and related complications were also improved. A longer follow-up period is needed.

BACKGROUND: Metabolic syndrome refers to risk factors for cardiovascular disease. Hyperglycemia is a critical component contributing to the predictive power of the syndrome. This study aimed to evaluate the results from the laparoscopic interposition of an ileum segment into the proximal jejunum for the treatment of metabolic syndrome in patients with type 2 diabetes mellitus and a body mass index (BMI) lower than 35. METHODS: Laparoscopic procedures were performed for 60 patients (24 women and 36 men) with a mean age of 51.7 +/- 6.4 years (range, 27-66 years) and a mean BMI of 30.1 +/- 2.7 (range, 23.6-34.4). All the patients had a diagnosis of type 2 diabetes mellitus (T2DM) given at least 3 years previously and evidence of stable treatment using oral hypoglycemic agents, insulin, or both for at least 12 months. The mean duration of type 2 diabetes mellitus was 9.6 +/- 4.6 years (range, 3-22 years). Metabolic syndrome was diagnosed for all 60 patients. Arterial hypertension was diagnosed for 70% of the patients (mean number of drugs, 1.6) and hypertriglyceridemia for 70%. High-density lipoprotein was altered in 51.7% of the patients and the abdominal circumference in 68.3%. Two techniques were performed: ileal interposition (II) into the proximal jejunum and sleeve gastrectomy (II-SG) or ileal interposition associated with a diverted sleeve gastrectomy (II-DSG). RESULTS: The II-SG procedure was performed for 32 patients and the II-DSG procedure for 28 patients. The mean postoperative follow-up period was 7.4 months (range, 3-19 months). The mean BMI was 23.8 +/- 4.1 kg/m(2), and 52 patients (86.7%) achieved adequate glycemic control. Hypertriglyceridemia was normalized for 81.7% of the patients. An high-density lipoprotein level higher than 40 for the men and higher than 50 for the women was achieved by 90.3% of the patients. The abdominal circumference reached was less than 102 cm for the men and 88 cm for the women. Arterial hypertension was controlled in 90.5% of the patients. For the control of metabolic syndrome, II-DSG was the more effective procedure. CONCLUSIONS: Laparoscopic II-SG and II-DSG seem to be promising procedures for the control of the metabolic syndrome and type 2 diabetes mellitus. A longer follow-up period is needed.

RESUMO

In this paper, findings are submitted from 11-yr bacteriological examination of children from the pre-school institutions in the city of Lviv. Stability was ascertained as was high level of nasopharyngeal carriage of the leading representatives of conditionally pathogenic microflora, an association was established to some disease forms. There is the need for the bacteria carriers to be identified in a purposeful and persevering fashion, with the relevant sanitary measures to be instituted in such individuals.
